Smart Goal Summer Development Camp is a structured development program for children ages 6–16, combining football training, cognitive growth, movement, social interaction, and meaningful daily activities in a safe, professional, and positive environment.

One of the major advantages of our program is flexibility and comfort.
Unlike fully outdoor camps that depend entirely on weather, our program combines both indoor and outdoor activities, allowing children to stay active and comfortable throughout the day.
On hot days, rainy days, or when indoor sessions are more effective, children continue training and learning in our indoor facilities without disruption.

Medication Administration Authorization
I authorize Smart Goal Academy staff to administer the following medication(s) to my child during training sessions, in accordance with the instructions provided.
All medications must be provided in original packaging with pharmacy label, clearly marked with the participant's name. Smart Goal Academy will not administer any medication without this signed authorization.

Authorized Pick-Up & Transportation
No participant will be released to any person not listed below. Government-issued photo identification will be required for verification. The registering parent/guardian is automatically authorized.
Transportation policy. Children ages 5–12 must be released exclusively to a parent/guardian or a pre-authorized adult listed below. No independent departure under any circumstances. Children ages 13–16 may be released to parent-authorized third-party transportation (e.g., taxi, rideshare) only with explicit written consent below. No participant will be permitted to leave the training venue on foot, bicycle, or public transit without express written parental consent, and only for ages 13+.
I understand and accept that Smart Goal Academy's duty of care ends at the point of verified release to an authorized person or upon the authorized independent departure of the participant. I assume full responsibility for any transportation-related risks following release.

Parent/Guardian Consent, Assumption of Risk & Release of Liability
PLEASE READ THIS SECTION CAREFULLY. IT CONTAINS A RELEASE OF LIABILITY AND ASSUMPTION OF RISK THAT AFFECTS YOUR LEGAL RIGHTS.
4.1 Nature of activities. Smart Goal Academy Outdoor Training Season operates from June 1 to September 15, 2026. The program includes, but is not limited to: outdoor soccer training sessions, technical and tactical drills, small-sided games, scrimmages, physical fitness exercises, agility and coordination exercises, and related athletic activities. Activities are conducted primarily outdoors at municipal parks and school fields in the City of Vaughan, Ontario.
4.2 Acknowledgment of inherent risks. I acknowledge and understand that participation in soccer and outdoor training activities involves inherent risks that cannot be fully eliminated, including but not limited to: physical contact, collisions with other participants, falls on playing surfaces (natural grass, artificial turf, uneven terrain), and ball impact injuries; muscle strains, sprains, fractures, ligament injuries, dental injuries, and concussions; heat-related illness (heat exhaustion, heat stroke, dehydration), sunburn, and weather-related exposure including thunderstorms and lightning; injury from equipment, field conditions, goal posts, fencing, or facility features; allergic reactions, insect bites and stings (including bees, wasps, ticks), and environmental hazards; communicable illness, including but not limited to COVID-19 and other respiratory infections; risks associated with outdoor training in varying weather conditions over an extended season; psychological stress, emotional distress, or anxiety associated with competitive or group activities.
4.3 Voluntary assumption of risk. I voluntarily assume all risks associated with my child's participation in the Smart Goal Academy Outdoor Training Season, including those described above and any risks not specifically identified. I confirm that my child is physically fit and medically cleared to participate, and that I have disclosed all relevant medical information.
4.4 Release of liability. To the fullest extent permitted by the laws of the Province of Ontario and applicable federal legislation, I, on behalf of myself, my child, and our respective heirs, executors, administrators, and assigns, hereby release, waive, and forever discharge Smart Goal Academy and its directors, officers, employees, coaches, instructors, volunteers, agents, and independent contractors; the City of Vaughan, its employees, agents, and facility operators; and any facility owner, landlord, or property manager (collectively, the "Released Parties") from any and all claims, demands, actions, causes of action, losses, damages, costs, and liabilities of every kind and nature whatsoever, whether known or unknown, arising out of or in connection with my child's participation in the program, including claims arising from the ordinary negligence of the Released Parties.
4.5 Exclusions from release. This release does not apply to: gross negligence or wilful misconduct by any of the Released Parties; reckless disregard for participant safety; physical, sexual, or emotional abuse; any non-waivable duty imposed by Ontario or Canadian law, including obligations under the Occupiers' Liability Act, R.S.O. 1990, c. O.2.
4.6 Indemnification. I agree to indemnify, defend, and hold harmless the Released Parties from any and all claims, damages, losses, costs (including reasonable legal fees), and liabilities arising from my child's participation in the program or from any breach of the representations, warranties, or agreements contained in this document.
4.7 Authorization for emergency medical treatment. In the event of an injury, illness, or medical emergency, I authorize Smart Goal Academy and its staff to take all reasonable steps to secure emergency medical care for my child, including but not limited to calling emergency medical services (911), transporting to the nearest hospital, and authorizing necessary medical treatment. I understand that I will be responsible for all costs associated with emergency medical treatment that are not covered by OHIP or private insurance.
4.8 Governing law. This agreement shall be governed by and construed in accordance with the laws of the Province of Ontario and applicable laws of Canada. The parties agree to submit to the exclusive jurisdiction of the courts of the Province of Ontario.
4.9 Severability. If any provision of this agreement is found to be invalid, illegal, or unenforceable by a court of competent jurisdiction, such finding shall not affect the remaining provisions, which shall continue in full force and effect.
4.10 Entire agreement. I confirm that I have read and understood all sections of this document. I am signing voluntarily and without duress. I understand that by signing, I am giving up substantial legal rights, including the right to sue. I have had the opportunity to seek independent legal advice before signing.

Concussion Awareness & Acknowledgment
Pursuant to Rowan's Law (Concussion Safety), 2018, S.O. 2018, c. 1 and Ontario Regulation 161/19.
I confirm that I have reviewed and understand the Concussion Awareness Resources as required under Rowan's Law (Concussion Safety), 2018, including the Government of Ontario's concussion awareness resources applicable to my child's age group.
I understand and agree to the following:
• A concussion is a serious brain injury that requires immediate attention and may have long-term health consequences if not properly managed.
• If my child is suspected of having sustained a concussion during any training session, they will be immediately removed from participation and will not be permitted to return to activity that day.
• My child will not be permitted to return to full participation until they have been evaluated and cleared in writing by a physician or nurse practitioner, in accordance with Rowan's Law and applicable regulations.
• I am responsible for monitoring my child for concussion symptoms following any head impact incident reported by coaching staff.
• I will promptly inform Smart Goal Academy of any concussion diagnosis, treatment, or medical clearance.

Code of Conduct & Safe Sport Policy
Participant Code of Conduct. All participants in the Smart Goal Academy Outdoor Training Season are expected to: treat all coaches, staff, volunteers, and fellow participants with respect, dignity, and fairness at all times; refrain from any form of bullying, harassment, discrimination, intimidation, physical or verbal aggression, abusive language, or threatening behaviour; follow all safety instructions and training rules as communicated by coaching staff; use all equipment and facilities in a safe and responsible manner; refrain from bringing any prohibited items to training, including weapons, drugs, alcohol, tobacco/vaping products, or any item that may endanger the safety of others; demonstrate good sportsmanship, fair play, and positive attitude during all activities; refrain from any unauthorized use of mobile phones, recording devices, or social media during training sessions.
Anti-Harassment & Discrimination Policy. Smart Goal Academy maintains a zero-tolerance policy for harassment and discrimination of any kind, consistent with the Ontario Human Rights Code, R.S.O. 1990, c. H.19. No participant, parent, coach, or staff member shall engage in conduct that discriminates on the basis of race, ancestry, place of origin, colour, ethnic origin, citizenship, creed, sex, sexual orientation, gender identity, gender expression, age, record of offences, marital status, family status, or disability.
Disciplinary measures. Smart Goal Academy reserves the right to take the following measures in response to violations: verbal warning and discussion with participant; temporary removal from activity (cooling-off period); contact with parent/guardian for immediate pickup; suspension from upcoming training sessions without refund; permanent dismissal from the program without refund. Serious safety violations, acts of violence, or any form of abuse may result in immediate dismissal without prior warning. Smart Goal Academy will cooperate with law enforcement authorities as required.
Parent/Guardian conduct. Parents and guardians are also expected to conduct themselves respectfully toward all staff, coaches, volunteers, and other families. Abusive, threatening, or disruptive behaviour by a parent/guardian may result in the participant's removal from the program.

Photo, Video & Media Consent
Smart Goal Academy may photograph or record video of training activities for promotional, educational, website, social media, and archival purposes. Images may be used on the Smart Goal Academy website (smartgoal.ca), Instagram, Facebook, YouTube, printed marketing materials, and partner communications.
This consent may be revoked at any time by providing written notice to Smart Goal Academy at info@smartgoal.ca.

Payment, Cancellation & Refund Policy
Registration fee. The registration fee for the Smart Goal Academy Outdoor Training Season (June 1 – September 15, 2026) includes coaching instruction, training equipment use, and seasonal programming. It does not include personal equipment, meals, or transportation.
Pricing structure. Season fee: $900 CAD per participant for the full season. Sibling discount: a 10% discount is applied to the registration fee for each additional sibling enrolled in the same season. The discount is applied to the second and each subsequent sibling. Discounts are not cumulative; where multiple discounts may apply, the most favourable rate will be used.
Payment terms. Full payment is required at the time of registration to secure enrollment. Registration is not confirmed until payment is received in full. Payment plans may be arranged on a case-by-case basis at the discretion of Smart Goal Academy.
Cancellation & refund. Cancellation 30+ calendar days before the season start date (June 1): full refund minus a $50 administrative fee. Cancellation 14–29 calendar days before the season start date: 50% refund. Cancellation less than 14 calendar days before the season start date: no refund. No refunds will be issued for missed sessions, early withdrawal, or dismissal due to conduct violations. Smart Goal Academy reserves the right to cancel or modify the program due to insufficient enrollment, facility unavailability, or force majeure; in such cases, a pro-rated refund will be provided for the remaining unused portion of the season.
Season interruption. In the event of prolonged facility closures, public health restrictions, or other circumstances beyond Smart Goal Academy's reasonable control that result in the cancellation of four (4) or more consecutive scheduled training sessions, Smart Goal Academy will offer affected participants a pro-rated credit toward future programming or a partial refund at its discretion.

Privacy Policy & Data Protection
Smart Goal Academy collects personal information in this registration package for the following purposes: administration of seasonal registration and program delivery; emergency contact and medical care; communication with parents/guardians regarding training schedules, locations, and program updates; and compliance with applicable laws and regulations.
Personal information is collected under the authority of the Personal Information Protection and Electronic Documents Act (PIPEDA) and applicable Ontario privacy legislation. Information will not be shared with third parties except as required for emergency medical care, legal compliance, or with explicit consent.
Parents/guardians may request access to, correction of, or deletion of their child's personal information by contacting Smart Goal Academy at info@smartgoal.ca.
All participant records will be retained for a period of seven (7) years following the last date of participation, after which they will be securely destroyed.
Illness, Communicable Disease & Health Screening
To protect all participants, staff, and families, the following health policies apply:
• No participant shall attend training if they are experiencing fever (38°C / 100.4°F or higher), vomiting, diarrhea, or symptoms of communicable illness.
• Parents/guardians must notify Smart Goal Academy immediately if their child has been diagnosed with or exposed to a communicable disease.
• Smart Goal Academy reserves the right to conduct visual health screenings upon arrival and to refuse entry to any participant who appears symptomatic.
• If a participant becomes ill during a training session, the parent/guardian will be contacted for immediate pickup. The participant must be picked up within one (1) hour of notification.
• A participant who has been sent home due to illness may not return until symptom-free for a minimum of 24 hours without medication, or until medical clearance has been provided.
• Smart Goal Academy will follow all applicable public health directives issued by York Region Public Health or the Province of Ontario.
Weather, Heat & Sun Safety; Attendance & Late Pick-Up
Weather, heat & sun safety. Smart Goal Academy prioritizes participant safety in all weather conditions. Given the extended outdoor season (June through September), participants will be exposed to varying weather:
• All participants must arrive with sunscreen applied (SPF 30+), a water bottle (minimum 500 ml), and a hat/cap. Reapplication of sunscreen will be encouraged throughout the session.
• Mandatory hydration breaks will be taken at minimum every 20–30 minutes during outdoor activities.
• Activities will be modified or cancelled when the Humidex exceeds 40°C or the UV Index is rated "Extreme" by Environment Canada.
• In the event of a thunderstorm, lightning, or severe weather warning, all outdoor activities will be immediately suspended. Participants will be directed to shelter or parents/guardians will be contacted for pickup.
• Training sessions cancelled due to severe weather will be rescheduled where possible. No refunds will be issued for individual weather-related cancellations.
• Coaches will monitor real-time weather conditions and have the authority to modify, relocate, or cancel any session in the interest of participant safety.
Attendance. Participants are expected to attend all scheduled training sessions. Parents/guardians are responsible for informing coaching staff of any planned absences in advance. No refunds or credits will be issued for individual missed sessions.
Late pick-up. Training sessions conclude at the designated time as communicated in the seasonal schedule. Parents/guardians are expected to arrange timely pick-up.
• A grace period of 10 minutes is provided after the scheduled end time.
• After the grace period, a late fee of $1.00 per minute will be applied.
• If a parent/guardian cannot be reached within 30 minutes past the scheduled end time, emergency contacts will be called.
• If no authorized person can be reached within 60 minutes, Smart Goal Academy may be required to contact the appropriate child welfare authorities, consistent with Ontario's Child, Youth and Family Services Act, 2017, S.O. 2017, c. 14, Sched. 1.
• Persistent late pick-ups may result in a discussion with the parent/guardian and, if unresolved, may result in the participant's removal from the program.

• I am the parent or legal guardian of the participant named in this document.
• I have read, understood, and agree to all sections of this Smart Goal Academy Outdoor Training Season Legal Protection Package, including the Assumption of Risk, Release of Liability, Indemnification, Code of Conduct, Concussion Acknowledgment, Media Consent, Refund Policy, Privacy Policy, and all other terms and conditions.
• All information provided in this document is true, accurate, and complete to the best of my knowledge.
• I understand that providing false or incomplete medical information may endanger my child and may limit Smart Goal Academy's ability to provide appropriate care.
• I have had the opportunity to ask questions and seek independent legal advice before signing this document.
• I am signing this document voluntarily and without duress.
