2026 – 2027 One Lesson Registration

PLEASE READ THIS SECTION CAREFULLY.

2.1 Nature of Activities

The program operates from September 20, 2026 – June 11, 2027. It includes outdoor and indoor soccer and futsal training, parent-child sessions (for U4), technical and tactical drills, small-sided games, scrimmages, physical fitness exercises, and related activities at venues in the City of Vaughan, Ontario.

For U4 participants (ages 3–4), a parent or guardian must remain present for the entire duration of each session.

2.2 Acknowledgment of Inherent Risks

I acknowledge that participation involves inherent risks that cannot be fully eliminated, including but not limited to:

Outdoor Risks:

• Physical contact, collisions, falls on natural grass, artificial turf, or uneven terrain
• Heat-related illness, sunburn, weather exposure including thunderstorms and lightning
• Insect bites and stings, environmental allergens, and other outdoor hazards

Indoor Risks:

• Collisions with walls, goal posts, gymnasium equipment, bleachers, or pillars
• Falls on indoor surfaces; ball impact at close range in confined spaces
• Indoor air quality, ventilation limitations, and wet surfaces near entrances during winter

Year-Round Risks:

• Muscle strains, sprains, fractures, ligament injuries, dental injuries, and concussions
• Communicable illness, including COVID-19 and other respiratory infections
• Emotional distress or anxiety associated with competitive or group activities
• Travel to and from training venues in adverse weather

Additional Considerations for U4 (Ages 3–4):

• Developmental variability in motor skills, coordination, and attention span
• Separation anxiety or emotional distress; toileting needs remain parent’s responsibility
• Greater vulnerability to temperature extremes, dehydration, and fatigue

2.3 Voluntary Assumption of Risk

I voluntarily assume all inherent risks associated with my child’s participation. I confirm that my child is physically able to participate and that I have disclosed all relevant medical and health information.

2.4 Consent to Participation

I consent to my child’s participation in the Smart Goal Academy program described in Section 1.

3.1 Release of Parent’s/Guardian’s Own Claims

To the fullest extent permitted by applicable law, I, in my own personal capacity (not on behalf of my child), hereby release, waive, and discharge:

• Smart Goal Academy and its directors, officers, employees, coaches, instructors, volunteers, agents, and independent contractors
• The City of Vaughan, its employees, agents, and facility operators
• Any facility owner, landlord, or property manager

(collectively, the “Released Parties”) from any and all claims that I personally may have arising from my child’s participation, including claims arising from ordinary negligence.

3.2 Indemnification

I agree to indemnify the Released Parties from claims arising from:

• My own acts or omissions
• Materially false or incomplete information provided by me
• My breach of any obligation in this Package

to the extent permitted by applicable law.

3.3 Limitation of Release

• Gross negligence or wilful misconduct
• Reckless disregard for participant safety
• Physical, sexual, or emotional abuse

3.4 Preservation of Minor’s Rights

Nothing in this Agreement is intended to waive, release, compromise, or otherwise limit any right or claim of the minor participant that a parent or guardian cannot lawfully waive, release, compromise, or limit under applicable law.

3.5 General Limitation

Nothing in this Agreement excludes or limits liability to the extent such liability cannot lawfully be excluded or limited under applicable law.

3.6 Emergency Medical Authorization

I authorize Smart Goal Academy to secure emergency medical care for my child, including calling 911 and authorizing necessary treatment.

3.7 Governing Law

This Agreement is governed by the laws of Ontario and Canada. If any provision is found unenforceable, the remaining provisions continue in full force.

4.3 Medical Disclosure

I confirm that all information above is true and complete.

4.4 Medication Administration

SGA does not routinely administer medication. Where required, contact the Head Coach for individual arrangements. Emergency medications must be provided in original packaging with the participant’s name.

Pursuant to Rowan’s Law (Concussion Safety), 2018, S.O. 2018, c. 1 and Ontario Regulation 161/19.

5.1 Concussion Awareness Resource Confirmation

I confirm that, before registration and within the previous 12 months, I have reviewed the applicable Ontario Concussion Awareness Resource, as required where Rowan’s Law and O. Reg. 161/19 apply.

Where required, the participant also confirms review of the age-appropriate resource within the previous 12 months.

5.2 SGA Concussion Code of Conduct

Statutory commitments:

• Fair play and respect for all participants, coaches, officials, and opponents.
• Recognition and reporting of possible concussion symptoms, including self-reporting.
• Supporting and complying with the graduated Return-to-Sport process.
• Sharing pertinent concussion information with the athlete’s school and other sport organizations.

As a condition of participation, I agree to:

• Immediately report any suspected concussion symptoms to coaching staff.
• Support immediate removal of my child from activity when a concussion is suspected.
• Refrain from pressuring coaching staff to return my child prematurely.
• Comply with SGA’s Return-to-Sport Protocol.
• Promptly inform SGA of any concussion diagnosis, treatment, or clearance.

5.3 Removal-from-Sport Protocol

Designated Person: The Head Coach or designated adult administers removal/return protocols.

• Immediate removal from activity upon suspected concussion.
• No return to physical activity for the remainder of that session.
• Monitoring until released to parent/guardian or emergency contact.
• Written Concussion Incident Report provided to parent for their healthcare provider.
• Record of every removal retained securely by SGA.

5.4 Return-to-Sport Protocol

If not diagnosed with concussion: return permitted after medical assessment and written clearance.

If diagnosed: graduated return-to-sport stages must be completed with required medical clearance before unrestricted participation. All documentation submitted to and reviewed by the Head Coach.

5.6 Concussion Information Privacy

Information collected through removal/return protocols is limited to what is reasonably necessary, with access limited to persons performing their duties. Records retained per Section 9.

6.1 Participant Expectations

• Treat all coaches, staff, volunteers, and participants with respect.
• No bullying, harassment, discrimination, intimidation, aggression, or abusive language.
• Follow all safety instructions and training rules.
• Use all equipment and facilities safely.
• No prohibited items (weapons, drugs, alcohol, tobacco/vaping).
• Appropriate footwear: cleats outdoor, non-marking indoor. Shin guards for match-play (U6+). Soft-soled shoes for U4.
• Good sportsmanship, fair play, positive attitude.
• No unauthorized mobile phones or recording devices during sessions.

6.2 Anti-Harassment & Discrimination

Zero-tolerance policy consistent with the Ontario Human Rights Code, R.S.O. 1990, c. H.19.

6.3 Parent/Guardian Conduct

Respectful behaviour required. Abusive or threatening behaviour may result in participant’s removal.

6.4 Disciplinary Procedure

• Verbal warning.
• Temporary removal from activity.
• Contact with parent for immediate pickup.
• Suspension (written decision with reasons for suspensions exceeding 2 weeks).
• Permanent dismissal (written decision with reasons, except for immediate safety removal).

SGA will consider a reasonable credit or pro-rated refund for unused pre-paid fees, except for serious misconduct, violence, abuse, or Safe Sport violations.

Smart Goal Academy is committed to a safe, inclusive, and respectful environment for all.

7.1 Rule of Two

No coach/volunteer shall be alone with a minor in a private or closed setting. Minimum two screened adults, or one screened adult in an observable and open environment.

7.2 Screening

All coaches and volunteers with direct minor interaction must hold a current PVSC, renewed every 3 years.

7.3 Reporting & Complaints

Reports of harassment, abuse, or maltreatment to the Head Coach or Board of Directors (info@smartgoal.ca). SGA will investigate promptly and confidentially, using independent investigators where appropriate, and will not interfere with police or child-protection investigations.

7.4 Electronic Communication

Coaches shall not communicate with minors via personal social media or direct messages. All communication through parents/guardians or official SGA channels.

7.5 Change Rooms & Photography

No photography or mobile devices in change rooms or washrooms.

7.6 Physical Contact

Limited to instruction, demonstration, safety, or injury response. Appropriate, brief, and in an open setting.

The Smart Goal Academy Once-Per-Week Program runs from September 20, 2026 to June 11, 2027.

8.1 What’s Included

• One training session per week
• Player assessment and progress monitoring throughout the season

8.2 Season Fee

Full Season — $1,500

One payment of $1,500 CAD for the entire season, payable before the participant’s first regular session unless otherwise agreed in writing.

A $100 discount applies when payment is made by cash or e-transfer: $1,400 CAD.

This fee guarantees your child’s place in the program for the full season.

8.3 Sibling Discount

10% discount for each additional sibling enrolled in the same season, beginning with the second child.

8.4 Joining Mid-Season

New players welcome throughout the year, subject to availability. Fees for the remaining season are calculated on a pro-rated basis.

Every new player is welcome to attend a FREE TRIAL SESSION before registration.

8.5 Cancellation & Refund

• 30+ days before season start — full refund less $75 admin fee.
• 14–29 days before season start — 50% refund.
• Less than 14 days — no refund.
• Medical withdrawal (physician’s letter): pro-rated refund may be considered, less $75 admin fee.
• SGA cancellation: pro-rated refund for remaining unused portion.

Nothing in this Package limits, waives, or contracts out of any right or remedy available under applicable law, including the Consumer Protection Act, 2002.

Smart Goal Academy is committed to protecting personal information in accordance with applicable Canadian privacy laws and recognized privacy principles.

9.1 Information We Collect

• Names, contact information, date of birth
• Emergency contacts
• Medical/health information relevant to safe participation
• Payment and billing
• Photographs/video (with consent)
• Attendance and assessment records

9.2 Purpose

Registration, delivery, emergency care, communication, assessment, billing, legal compliance.

9.3 Consent

By signing this Package, you consent to collection, use, and disclosure for purposes above. Withdrawal: info@smartgoal.ca.

9.4 Limiting Collection

Only reasonably necessary information collected. No use beyond stated purposes without additional consent.

9.5 Disclosure

Not shared except: emergency medical care; law (including child protection); service providers (under confidentiality); your explicit consent.

9.6 Safeguards

Reasonable administrative, technical, and physical safeguards. Digital records stored securely with limited access.

9.7 Access & Correction

Contact info@smartgoal.ca. Response within 30 days.

9.8 Records Retention

• Waivers, consent, incident reports: 7 years after last participation.
• Medical/emergency contacts: end of last season or as required.
• Payment/billing: 7 years.
• Assessment records: 3 years.
• Media consent: until withdrawn.

Secure destruction upon expiry.

9.9 Breach Response

Where PIPEDA applies: notification and reporting as required. Where another regime applies: compliance with corresponding requirements.

Please indicate consent for each category:

Incidental appearance in wide-angle or group imagery may not always be preventable, but SGA will not intentionally publish an identifiable close-up without consent.

11.1 Authorized Persons

Photo ID required for verification.

11.2 Temporary Pick-Up

Authorized parent/guardian may provide temporary authorization via verified email or text to staff, including the person’s full name. Photo ID required.

11.3 Age-Based Rules

• Ages 3–4 (U4): Parent/guardian must remain present for the entire session.
• Ages 5–12: Released only to authorized or temporarily authorized adult.
• Ages 13–16: Third-party or independent departure only with written consent below.

11.4 Third-Party / Independent Departure (Ages 13–16)

SGA’s supervision responsibility ordinarily ends once the participant has been released in accordance with this Policy.

• 10-minute grace period after scheduled end.
• After grace: $1.00/minute late fee may apply.
• If no contact reachable within reasonable time, SGA may contact appropriate authorities where reasonably necessary to protect the participant.

Persistent late pick-ups may affect continued enrollment.

13.1 Outdoor (Sep–Nov, Apr–Jun)

• Sunscreen, water, hat recommended.
• Hydration breaks during activities.
• SGA may modify, relocate, or cancel based on temperature, Humidex, UV, air quality, field conditions, and other safety considerations.
• Immediate suspension during thunderstorms or lightning.

13.2 Indoor (Nov–Mar)

• Non-marking footwear; shin guards for match-play (U6+).
• Pre-session inspection.
• Sessions may be modified for ventilation or heat concerns.

13.3 Winter Weather

• Cancellations during severe weather warnings communicated via email/social media as early as possible.
• No refunds for weather-related cancellations.

13.4 Emergencies

Follow host facility procedures. Headcount at assembly points.

13.5 Illness

• No attendance with fever 38°C+, vomiting, diarrhea, or communicable illness.
• Pickup within 1 hour of notification.
• Return after 24h symptom-free or with clearance.
• SGA follows York Region Public Health and provincial directives.

By signing below, I confirm:

• I am the parent or legal guardian of the participant.
• I have read, understood, and agree to all sections (1–13).
• I was provided access to the complete Package before signing and had reasonable opportunity to review it.
• All information I provided is true, accurate, and complete.
• I have had the opportunity to seek independent legal advice.
• I am signing voluntarily and without duress.
• I understand that by signing, I am giving up certain legal rights as described in Sections 2 and 3.
• This Package is Version 3.0. Any material amendment after signing will be communicated to me and, where additional consent is legally required, will not take effect against me until obtained.
• Nothing in this Package is intended to waive or contract out of any right that applicable law prohibits the parties from waiving.
