Consent, Assumption of Risk & Release
3.1 Nature of Activities
Smart Goal Community Game Day is a free, weekly recreational soccer program for children and youth. Activities include recreational soccer games, small-sided matches, and related physical activities at an indoor facility in North Toronto. Coaching is provided on a volunteer basis.
3.2 Acknowledgment of Inherent Risks
I acknowledge that participation involves inherent risks including:
- Physical contact, collisions, falls, and ball impact
- Muscle strains, sprains, fractures, and concussions
- Collisions with walls, goal posts, or gymnasium equipment
- Communicable illness in enclosed indoor spaces
- Emotional distress associated with group or competitive activities
3.3 Voluntary Assumption of Risk
I voluntarily assume all inherent risks. I confirm that my child is physically able to participate and that I have disclosed all relevant medical information.
3.4 Release of Parent’s/Guardian’s Own Claims
To the fullest extent permitted by law, I, in my own personal capacity (not on behalf of my child), release Smart Goal Academy, its directors, officers, volunteers, coaches, agents, and the facility owner/operator from claims that I personally may have arising from my child’s participation, including claims arising from ordinary negligence.
This release does not apply to gross negligence, wilful misconduct, reckless disregard, or abuse.
3.5 Preservation of Minor’s Rights
Nothing in this Agreement is intended to waive, release, compromise, or otherwise limit any right or claim of the minor participant that a parent or guardian cannot lawfully waive under applicable law.
3.6 General Limitation
Nothing in this Agreement excludes or limits liability to the extent such liability cannot lawfully be excluded or limited under applicable law.
3.7 Emergency Medical Authorization
I authorize SGA to secure emergency medical care for my child, including calling 911 and authorizing necessary treatment.

Concussion — Rowan’s Law
Pursuant to Rowan’s Law (Concussion Safety), 2018, S.O. 2018, c. 1.
I confirm that I have reviewed the applicable Ontario Concussion Awareness Resource within the previous 12 months, as required where Rowan’s Law applies.
I agree to:
- Immediately report suspected concussion symptoms to coaching staff.
- Support immediate removal from activity when a concussion is suspected.
- Not pressure coaches to return my child prematurely.
- Comply with any return-to-sport requirements (written medical clearance from a physician or nurse practitioner).
Code of Conduct & Safe Sport
5.1 Participant Expectations
- Respect all coaches, volunteers, and participants.
- No bullying, harassment, discrimination, or aggression.
- Follow all safety instructions.
- Appropriate indoor footwear (non-marking soles). Shin guards recommended.
- Good sportsmanship and fair play.
5.2 Parent/Guardian Conduct
Respectful behaviour required. Abusive or threatening behaviour may result in the participant being asked to leave.
5.3 Safe Sport
- Rule of Two: no coach/volunteer alone with a child in a private setting.
- All coaches/volunteers hold current Police Vulnerable Sector Check.
- No photography or mobile devices in change rooms or washrooms.
- Reports of any concern: info@smartgoal.ca or to any coaching staff on site.
Pick-Up & Supervision
Participants must be picked up promptly at the end of their session (7:00 PM or 8:00 PM).
6.1 Authorized Persons
The registering parent/guardian is automatically authorized. Additional authorized persons may be listed below.
6.2 Age-Based Rules
- Ages 5–12: Released only to authorized adult.
- Ages 13+: May depart independently only with written consent below.
SGA’s supervision responsibility ordinarily ends once the participant has been released to an authorized person or permitted to depart independently.
If no authorized person can be reached within a reasonable time, SGA may contact appropriate authorities where necessary to protect the participant.

Media Consent
SGA may photograph or record Community Game Day activities. Consent does not affect participation.
Incidental appearance in group imagery may not always be preventable.

Privacy
SGA collects personal information in this form for registration, emergency care, and communication. Information is protected in accordance with applicable Canadian privacy laws, not shared with third parties except for emergency care or legal requirements, and securely destroyed when no longer needed. - I am the parent or legal guardian of the participant.
- I have read, understood, and agree to all sections (1–8).
- All information I provided is true and complete.
- I am signing voluntarily.
- I understand that by signing, I am giving up certain legal rights as described in Section 3.
