Arrival, Hand-off, Pick-up & Transportation
3.1 When SGA Supervision Begins and Ends
- SGA supervision begins only when the participant is personally received by the assigned coach or another authorized SGA representative at the designated training area at the scheduled time.
- SGA supervision ordinarily ends when the participant is personally released to the parent/guardian, Local Responsible Adult, or another Authorized Pick-Up Person, or when a participant age 13+ departs independently under the written authorization below.
- Dropping a child at a parking lot, curb, building entrance, lobby or field without a confirmed hand-off does not transfer supervision to SGA.
- The adult delivering the participant should remain until the coach has acknowledged receipt of the participant.
- SGA is not responsible for transportation arranged independently by the parent/guardian before the participant is received or after the participant is released.
- A participant will not be released to an unauthorized person.
3.2 Authorized Pick-Up / Driver Persons
Verification: SGA may require government-issued photo identification before releasing the participant. SGA may refuse release if identity or authorization cannot reasonably be verified.
3.3 Temporary Pick-Up Authorization
A temporary pick-up person must be authorized before release by the parent/guardian or Local Responsible Adult through a communication method reasonably verifiable by SGA (for example, the registered email address or phone number). The message should include the person’s full name and phone number. Photo ID may be required.

Late Pick-Up, No-Show & Unexpected Circumstances
- If an authorized adult is late, SGA will attempt to contact the Local Responsible Adult, parent/guardian and emergency contacts using the information provided.
- The participant will not be released to an unknown or unauthorized person merely because the scheduled pick-up person is delayed.
- The participant must not be instructed to wait alone outside the facility, in a parking lot or at another unapproved location.
- If SGA cannot reach any responsible adult and circumstances create a safety concern, SGA may contact facility security, police, emergency services or another appropriate authority.
- Repeated late pick-up, failed hand-off, inability to reach a responsible adult, or breach of safeguarding requirements may result in suspension or termination of individual training until a safe arrangement is established.
- If a participant does not arrive and no cancellation notice is received, SGA may contact the parent/guardian or Local Responsible Adult to confirm the participant’s status. SGA does not assume responsibility for locating a participant who was never received by SGA.

Medical Information & Emergency Authorization
5.1 Medical Disclosure
Parent/guardian responsibility: I will promptly update SGA if this information changes. I understand that incomplete or inaccurate information may affect SGA’s ability to respond appropriately.
5.2 Medication
SGA does not routinely administer medication. Emergency medication should be supplied in accordance with the participant’s established medical plan and applicable law. The parent/guardian and Local Responsible Adult remain responsible for ensuring that required medication is available, current and appropriately identified.
5.3 Emergency Response
I authorize SGA to take reasonable emergency steps when immediate action appears necessary, including stopping participation, providing basic first aid within the responder’s training, contacting 911, contacting the facility, and seeking emergency medical assistance. SGA will make reasonable efforts to contact the parent/guardian and Local Responsible Adult as soon as practicable.
SGA coaches are not acting as physicians or diagnosticians. Decisions are made based on observable circumstances, information provided to SGA, applicable protocols, and the need to protect the participant from immediate harm.

Private Training Safeguarding Protocol
These requirements apply to individual training involving a minor participant.
- Sessions should be conducted in an observable and interruptible environment, such as an open field, gym or training area where appropriate adults or facility users may reasonably be present or nearby.
- For younger participants or where required by SGA, the parent/guardian, Local Responsible Adult or another authorized adult must remain at the facility or in the designated vicinity for the duration of the session.
- Private changing, showering or toileting assistance is not part of the coaching service. Coaches must not enter a change room with a participant except where an immediate safety emergency requires intervention.
- Physical contact must be limited to what is reasonably necessary for instruction, demonstration, first aid or immediate safety, and should be explained where practicable.
- Coaches must not transport a participant in a personal vehicle as part of the training service unless a separate written SGA transportation authorization has been approved in advance.
- Communication about scheduling, payment, injuries, incidents or significant concerns should include the parent/guardian or Local Responsible Adult and should use SGA-approved communication channels.
- No secret, disappearing or intentionally concealed communications with a minor participant are permitted.
- Photography or video must comply with Section 11. Recording in change rooms or washrooms is prohibited.
- SGA may require an additional adult, alter the session location, postpone a session or end a session if the safeguarding conditions are not adequate.
6.1 Coach Authority to Stop a Session
The coach or SGA may immediately pause or terminate a session where continued participation is reasonably considered unsafe or inappropriate, including suspected injury or concussion, illness, unsafe behaviour, aggression, harassment, safeguarding concerns, severe weather, unsafe playing conditions, facility directions, or failure to comply with the hand-off or supervision requirements.

Assumption of Risk & Parent/Guardian Release
PLEASE READ CAREFULLY.
7.1 Nature of Activities
Individual soccer training may include technical and tactical instruction, ball work, shooting, passing, movement, agility, strength/fitness activities, decision-making exercises and related athletic activity at indoor or outdoor facilities.
7.2 Inherent Risks
- Physical exertion, fatigue, muscle strains, sprains, fractures and other musculoskeletal injury.
- Concussion, head impact and ball impact.
- Falls, slips, collisions with participants, walls, goal posts, equipment or playing surfaces.
- Outdoor weather, heat, cold, sun exposure, poor air quality and changing field conditions.
- Indoor surface, equipment, ventilation and facility-related hazards.
- Communicable illness and other risks inherent in group/public sport environments.
- Risks that may arise despite reasonable supervision, instruction and safety precautions.
7.3 Voluntary Participation and Assumption of Inherent Risk
I understand the nature of the activities and voluntarily permit my child to participate. I confirm that, to the best of my knowledge, my child is able to participate and that I have disclosed information reasonably relevant to safe participation. I accept the inherent risks associated with the activity.
7.4 Release of Parent/Guardian’s Own Claims
To the fullest extent permitted by applicable law, I, in my own personal capacity and not on behalf of the minor participant, release Smart Goal Academy, its directors, officers, employees, coaches, volunteers and agents from claims that I personally may have arising from the participant’s involvement in the program, including claims arising from ordinary negligence, except to the extent such claims cannot lawfully be released.
7.5 Minor’s Rights Preserved
Nothing in this Package is intended to waive, release, compromise or limit any right or claim of the minor participant that a parent or guardian cannot lawfully waive. Nothing excludes liability for gross negligence, wilful misconduct, abuse, reckless disregard, or any liability that applicable law does not permit to be excluded or limited.
7.6 Facility and Third Parties
The participant may train at facilities not owned or controlled by SGA. SGA does not warrant the acts or omissions of independent facility owners/operators, transportation providers or other third parties, but this clause does not limit any duty or liability that applicable law imposes on SGA.

Concussion / Rowan’s Law Acknowledgment
SGA applies concussion-safety procedures to its sport programming. Where Rowan’s Law and its regulation apply, registration and participation are subject to the required annual review confirmations, SGA’s Concussion Code of Conduct, and SGA’s Removal-from-Sport and Return-to-Sport Protocols.
8.1 Removal From Sport
If SGA becomes aware that the participant has sustained or may have sustained a concussion, the participant will be removed from further training immediately in accordance with SGA’s protocol. The parent/guardian or Local Responsible Adult will be informed. The participant will not return to training merely because symptoms appear to improve during the session.
8.2 Medical Assessment and Return
A participant removed for a suspected concussion must follow SGA’s Return-to-Sport Protocol. Where medical assessment or medical clearance is required, SGA will require the applicable documentation before return. SGA may maintain records of removal and return-to-sport status as required by law and policy.

- Respect coaches, participants, facility staff and members of the public.
- Follow reasonable safety and facility instructions.
- Use appropriate footwear and equipment for the surface and activity.
- No violence, threats, harassment, bullying, discriminatory conduct, deliberate dangerous play or intentional damage.
- The parent/guardian and authorized adults must not interfere with emergency, safeguarding or facility procedures.
9.2 Incident Documentation
SGA may document injuries, suspected concussions, medical events, safeguarding concerns, unauthorized or disputed pick-up attempts, late pick-up, confrontations, significant behavioural incidents, emergency calls, facility incidents and other material events. Records may include the date, time, location, persons present, observations, actions taken, communications made and follow-up steps.
The parent/guardian and Local Responsible Adult agree to cooperate reasonably with incident documentation and follow-up. An acknowledgment of receipt of an incident report is not an admission of fault by any person.
9.3 Suspension / Termination for Safety
SGA may suspend or discontinue individual sessions where reasonably necessary to protect a participant, coach or other person, to comply with law or facility requirements, or where required safeguarding, medical, supervision or pick-up arrangements are not maintained.

Information may be disclosed where reasonably necessary for emergency care, safeguarding, insurance, legal obligations, facility requirements or other lawful purposes.
